Although great strides have been made to protect our patients from harm, they are still suffering health care-associated untoward events. Publically reported health care data shows us that patient harm is still occurring and hospitals are being financially penalized for less than optimal outcomes. Part of the issue is that we do not dig deep enough for the underlying solutions to these occurrences. Going deep into the root causes requires a new way of looking at things. We need to be brave, courageous, and bold. This conference will explore evidence in highly reliable care, workarounds, developments in fall reductions, the relationship between clinicians and medication systems, current competencies, and how to create a culture of safety.
